O fen\u00f4meno do envelhecimento \u00e9 natural e em grande parte geneticamente predisposto. As fun\u00e7\u00f5es da pele s\u00e3o cruciais para a homeostase e a sobreviv\u00eancia humana. Como o maior \u00f3rg\u00e3o do corpo humano, a pele, juntamente com a hipoderme (gordura subcut\u00e2nea), s\u00e3o a fonte e o alvo de v\u00e1rios horm\u00f4nios e neuromediadores, tornando-se um \u00f3rg\u00e3o end\u00f3crino perif\u00e9rico independente.   <\/p>\n<p>A pele tamb\u00e9m tem a capacidade de produzir o pr\u00f3-horm\u00f4nio vitamina D e transform\u00e1-lo em metab\u00f3litos ativos, que podem exercer diferentes efeitos em c\u00e9lulas importantes da pele (queratin\u00f3citos e fibroblastos) e c\u00e9lulas imunes (linf\u00f3citos), ativando o receptor nuclear de vitamina D (VDR).<\/p>\n<p>A vitamina D desempenha um papel fundamental na homeostase da pele, contribuindo para a sua fun\u00e7\u00e3o de barreira. Outra forma de pr\u00f3-vitamina D, o ergocalciferol, \u00e9 encontrada em quantidades muito pequenas em alimentos \u00e0 base de plantas. <\/li>\n<li>Atrav\u00e9s da exposi\u00e7\u00e3o ao sol, onde os f\u00f3tons da radia\u00e7\u00e3o UVB fotoisomerizam o 7-desidrocolesterol da epiderme em colecalciferol, que \u00e9 rapidamente convertido por uma rea\u00e7\u00e3o mediada pelo calor ao colecalficerol ou vitamina D3. Essas mol\u00e9culas passam por diferentes modifica\u00e7\u00f5es no f\u00edgado e nos rins para obter o metab\u00f3lito ativo. Entre as mol\u00e9culas envolvidas na regula\u00e7\u00e3o desse processo, encontramos os n\u00edveis de PTH, n\u00edveis de c\u00e1lcio e f\u00f3sforo e at\u00e9 TGF-beta.  <\/li>\n<\/ul>\n<p>Atualmente, existe uma defici\u00eancia subcl\u00ednica (30-50nmol\/L) e cl\u00ednica de vitamina D (&lt;30nmol\/L) na popula\u00e7\u00e3o em geral e isso se tornou um problema mundial. As necessidades di\u00e1rias de vitamina D dependem da idade e variam de acordo com diferentes consensos. A American Endocrine Society recomenda entre 400-1.000 UI\/dia no primeiro ano de vida e entre 600-1.000 UI\/dia acima dessa idade.  <\/p>\n<p>Uma popula\u00e7\u00e3o vulner\u00e1vel s\u00e3o os idosos, pois apresentam menor s\u00edntese cut\u00e2nea de 7-desidrocolesterol, diminui\u00e7\u00e3o da absor\u00e7\u00e3o intestinal de vitamina D secund\u00e1ria a menor n\u00famero de receptores de vitamina D nos enter\u00f3citos (c\u00e9lulas intestinais), al\u00e9m de menor capacidade de processamento da vitamina nos n\u00edveis hep\u00e1tico e renal.<\/p>\n<p><b>Fatores de risco para hipovitaminose D:<\/b><\/p>\n<div style=\"border: 1px solid black; padding: 20px; width: 60%; margin: 0 auto 40px;\">\n<ul>\n<li>Exposi\u00e7\u00e3o solar inadequada<\/li>\n<li>Tabagismo<\/li>\n<li>Obesidade<\/li>\n<li>Fototipos altos (IV ou superior)<\/li>\n<li>Idoso<\/li>\n<li>Institucionalizado<\/li>\n<li>Tratamento com corticoster\u00f3ides, antiepil\u00e9pticos<\/li>\n<\/ul>\n<\/div>\n<p>Muitos fatores ambientais contribuem para a defici\u00eancia de vitamina D, incluindo os meses de inverno, exposi\u00e7\u00e3o inadequada ao sol e localiza\u00e7\u00e3o em altas latitudes. A m\u00e1 exposi\u00e7\u00e3o ao sol atual devido ao confinamento e \u00e0 polui\u00e7\u00e3o do ar s\u00e3o os principais fatores que causam exposi\u00e7\u00e3o insuficiente aos raios UV. Por outro lado, poluentes org\u00e2nicos persistentes do ar e metais pesados podem se comportar como produtos qu\u00edmicos desreguladores end\u00f3crinos e podem causar defici\u00eancia de vitamina D direta ou indiretamente. Tendo em vista os benef\u00edcios t\u00f3picos da vitamina D, v\u00e1rios estudos est\u00e3o em andamento para desenvolver novos an\u00e1logos da vitamina D com um perfil cl\u00ednico mais seguro. <\/p>\n<p>Como os dermatologistas recomendam n\u00e3o a exposi\u00e7\u00e3o excessiva ao sol e contra-indicam o bronzeamento nas cabines, a visita dermatol\u00f3gica \u00e9 um momento crucial para abordar e informar sobre a exposi\u00e7\u00e3o solar de forma segura e limitada e, por outro lado, educar sobre a ingest\u00e3o diet\u00e9tica de vitamina D e fornecer recomenda\u00e7\u00f5es adequadas sobre a import\u00e2ncia da suplementa\u00e7\u00e3o oral de vitamina D.<\/p>\n<p>Essa interven\u00e7\u00e3o pode ser essencial, principalmente em pacientes com maior risco de desenvolver insufici\u00eancia ou defici\u00eancia de vitamina D.
